DSM-IVの診断基準を満たす血管性痴呆患者6例においてアニラセタム長期投与による症状の変化と、定量脳波・事象関連電位・局所脳血流の変化を比較検討した。アニラセタム1日量600mgを経口的に12週間連続投与した。薬物投与前、投与後8週、12週時に、症状評価スケール、知的機能検査、局所脳血流、事象関連電位および脳波を測定した。脳波は18部位からの単極導出記録を波形認識法を用いて定量解析した。事象関連電位は聴覚オドボール課題を用いた。結果は、投与前と12週後での症状評価スケール・知的機能検査では有意差は認められなかった。局所脳血流では脳血流の増加傾向が認められた。定量脳波ではシータ波の減少とアルファ波の増加が有意に認められた。事象関連電位ではP300の潜時が短縮、振幅の増加が有意に認められた。アニラセタムの効果を評価する客観的指標として定量脳波事象関連電位・局所脳血流の有用性が示唆された。

Changes in symptoms, quantitative electroencephalogram, event-related potential, and local cerebral blood flow due to long-term administration of aniracetam were studied comparatively in 6 cases of multi-infarct dementia fulfilling DSM-IV diagnostic criteria. Aniracetam was administered orally at 600mg/day for 12 continuous weeks. Symptom evaluation scales, a cognitive function battery, local cerebral blood flow, event-related potential, and EEG were measured prior to and at 8 weeks and 12 weeks following administration. Monopolar EEG records from 18 sites were analyzed quantitatively using brain wave recognition. Event-related potential was evaluated using audible hearing tasks. In the symptom evaluation scale and cognitive function battery results, no significant differences were observed in pre-administration and 12-week post-administration data. In local cerebral blood flow, an increase in cerebral blood flow was observed. In the quantitative EEG, a significant decrease in theta waves and a significant increase in alpha waves were observed. In event-related potential, P300 latency time was shortened, and amplitude increased significantly. These results suggest the utility of quantitative EEG event-related potential and local cerebral blood flow as objective indices for evaluation of aniracetam benefit.

今回われわれは1975年9月から1996年8月までの過去21年間に治療した下顎エナメル上皮腫一次症例29例中，保存的療法を施行した18例について検討を加えた．保存的療法は開窓3例，腫瘍摘出周囲骨削除14例，うち生検を兼ねた開窓が5例，開窓・摘出・反復処置が1例であった．Ｘ線像は単胞性が13例，多胞性が5例であった．組織型はplexiform typeが11例，follicular typeが3例，plexiform unicystic ameloblastomaが3例，不明が1例であった．再発は3例（16.7%）にみられ，そのＸ線像は単胞性が2例および多胞性が1例で，また組織型はplexiform typeが2例および不明が1例であった．再発の原因は正中部付近の腫瘍の見おとしが1例，下顎管や近心側歯胚周囲の腫瘍残存の2例が考えられた。

Among 29 primary cases of mandibular ameloblastoma treated in a prior 21-year interval from September, 1975-August, 1996, we conducted additional investigation of 18 cases in which conservative therapy was pursued. Conservative treatment consisted of 3 cases of marsupialization; 14 cases of bone curettage peripheral to the excised tumor, of which 5 were marsupialization accompanied by biopsy; and 1 case of marsupialization, excision, and repeated intervention. X-ray images were unicystic in 13 cases and multicystic in 5 cases. Tissue types were plexiform in 11 cases, follicular in 3 cases, plexiform unicystic ameloblastoma in 3 cases, and unknown in 1 case. Recurrence was seen in 3 cases (16.7%); their X-ray was unicystic in 2 cases and multicystic in 1 case, and their tissue type was plexiform in 2 cases and unknown in 1 case. Causes of recurrence were deemed an overlooked tumor near the original site in 1 case and residual tumors peripheral to the mandibular canal or the mesial tooth germ in 2 cases.
